Our Clients Agree in Writing
I understand and acknowledge that I have been advised to undergo a complete
physical examination prior to any workout session with Herzog Body Tech to indicate
whether or not I am physically capable of this activity. If at this time I do not have my
doctor's permission to exercise I acknowledge that I have scheduled or will schedule
a physical examination. Therefore I disclaim any liability or loss in connection with all
exercise and advice rendered by Herzog Body Tech and Greg Herzog.
In further consideration of the professional service rendered by Herzog Body Tech, I
hereby warrant and represent, I have been thoroughly informed by Herzog Body Tech
prior to the commencement of this exercise program concerning the degree of
difficulty and the possibility of my sustaining injuries as a result of the specialized
planned activities. However, while fully informed I hereby consent to participate.
I hereby represent and warrant to hold harmless, release and discharge, Herzog
Body Tech, Greg Herzog and any agent or employee of Herzog Body Tech, from all
potential claims and/or causes of action of any sort, for any and all injuries which I
may have sustained as a result of my participation in this exercise program.
I certify that my attendance in the above mentioned program is voluntary and of my
own free will.
I understand that there is a 48 hours cancellation policy, and if a 48 hours notice is
not given to Herzog Body Tech there will be a charge of that session's time slot.
